Care Plans

Care Plans are available for patients with a chronic disease. Chronic disease includes Diabetes, Asthma, Arthritis, Mental Health Issues, Heart Disease, Osteoporosis, Obesity and other long-lasting (6 months or more) conditions.

Care Plans help to ensure that all of your medical needs are being addressed and that you are actively involved with your own health care. Having a Care Plan also enables you to obtain Medicare rebates toward five allied health services per calendar year which can include physiotherapy, podiatry, counselling, exercise physiology and dieticians amongst others. A Care Plan lasts for 12 months and can be reviewed every 3 months.

Care Plans are actioned by your regular GP.
