Care Plans

These are available for patients with a chronic disease. They help to ensure that all your medical needs are being addressed, and that you are actively involved with your own health care. Having a care plan also enables you to get medicare rebates on up to 5 allied health services per calendar year, eg physiotherapy, podiatry, counselling, exercise physiology, dietician and many more. The care plan lasts for 12 months, and can be reviewed every 3 months. “Chronic disease” includes Diabetes, Asthma, Arthritis, Mental Health Issues, Heart Disease, Osteoporosis, Obesity, etc.
